PERINEOPLASTY

Perineoplasty restores aesthetic appearance of the vaginal vestibule and narrows the entrance to the vagina. The main indication for perineoplasty is stretching of the vaginal vestibule during childbirth and incision or rupture of the perineum. The procedure is performed under topical anaesthesia and takes several dozen minutes.
During the procedure the excess skin of the perineum is cut out, after which a physician brings the edges of the wound together and sutures it. As a result, the muscles building the vaginal walls come closer together, narrowing its lumen. The scars left by the perineal incision are removed, as well as any asymmetries. Absorbable sutures are used, so there is no need to remove them. After the procedure the Patient should remain under medical supervision for a few hours and then can return home.

IS THE TREATMENT APPROPRIATE FOR ME?

The primary indication for perineoplasty is natural childbirth, after which the perineal structure may become loose. During childbirth, muscles of the birth canal stretch considerably, and sometimes this excessive stretching of the perineum remains as an unwanted memorial after childbirth. An indication for perineoplasty is also an incision or rupture of the perineum during childbirth. Such an injury, when not perfectly sutured, can heal improperly. It may cause formation of a scar, feeling of pulling the fused tissue. In addition, an improperly healed perineum can look unsightly, with noticeable loosening of the vaginal vestibule and asymmetry. Presence of scar tissue may cause pain during the intercourse.
Perineoplasty is intended for women who want to improve appearance of the perineum. But perineoplasty is performed not only for aesthetic reasons. Loosening of the vaginal vestibule may be accompanied by a greater tendency to intimate infections, as well as vaginal dryness. Women who frequently experience vaginal inflammation may opt for perineoplasty for health reasons. Finally, the procedure may be performed in women whose vaginal dilatation negatively affects the quality of their sexual life. It happens that both the woman and her partner derive less pleasure from sex when the vaginal entrance is excessively loose.

PROCEDURE

During the consultation, we discuss the expected effect and chose the best method for the Patient. Before the procedure, topical anaesthesia or intravenous anaesthesia under the control of an anaesthesiologist is applied. A laser knife is used to make the cuts, which promotes rapid wound healing. Then sutures are applied.

EFFECT

The result of perineoplasty is narrowing of the entrance to the vagina. This produces an excellent aesthetic effect. It also has significant health benefits: reduced risk of reproductive tract infections. Another desirable effect of the procedure is elimination of scars after incision or perineal rupture, which increases comfort during intercourse, eliminates the feeling of pulling at the site of the scar and pain during intercourse. The benefit of perineoplasty is also improved quality of sexual life.
The final effect of this gynaecological surgery procedure can be assessed after the perineal area has healed, which is after about three months. After the procedure, the vaginal vestibule area may be swollen and reddened. The effect of the procedure is permanent unless there is another natural childbirth.

HOW SHOULD I PREPARE?

Since perineoplasty is a surgical procedure, a set of diagnostic tests must be performed before undergoing it. First of all, a blood count will be done to check for any abnormalities of the platelets, red blood cells and white blood cells. Another important test is determination of fasting glucose levels, as well as electrolyte, sodium and potassium, levels. Coagulation parameters APTT, PT and fibrinogen should be measured. Diagnostics for viral infections is also necessary: anti-HBS, HIV and HCV antigens are determined. In addition, blood creatinine levels and blood group can be determined. Biochemical tests should be performed no later than 2 weeks before the procedure, and serological tests – no later than three months before the planned perineoplasty. Before perineoplasty, a vaginal swab and culture are also required and you should have a current cytology result (done less than one year before the procedure).

RECOVERY

Perineoplasty does not require hospital stay after the procedure, after a few hours the Patient can go home. Recovery after perineoplasty takes about 4-8 weeks. Immediately after the surgery, the treatment site is swollen and red, and pain may occur. They disappear spontaneously after a few days. During the wound healing period, the Patient should maintain proper hygiene of the perineal area. This means that the wound should be washed with soap and water; this should be done in the morning and in the evening, as well as after each use of the toilet. We also recommend to air the perineum, which means staying without underwear as often as possible. If necessary, loose, airy underwear and clothing should be worn. During the first four to six weeks after the procedure the use of a swimming pool or sauna as well as intense physical activity should be avoided. Sexual intercourse may be resumed at the earliest 6-8 weeks after surgery.

CONTRAINDICATIONS

There are not many contraindications to perineoplasty, but they must be taken into consideration. Below you will find the most important of them.
Asymptomatic carrier of hepatitis B or hepatitis C.
Viral hepatitis often affects autoimmune reactions. It mainly depends on the stage of the disease as well as its activity. Only your attending physician may decide whether you are eligible for a aesthetic medicine procedure. Ask them for a written consent for the procedure. Only then, we can perform the procedure.
Diabetes.
In case of advanced diabetes wound healing is impaired. However, when treated and controlled, diabetes is not a problem. If you have diabetes, please ask your attending physician for a written consent for the aesthetic medicine procedure and present it to us on the day of the procedure.

Active cancer.

Cancer that occurred less than 5 years prior to the procedure.

Chemo- and radiotherapy

Autoimmune diseases apart from thyroid disease, acquired vitiligo, and alopecia areata. For example, these diseases include psoriasis, rheumatoid arthritis, lupus, myasthenia gravis, multiple sclerosis, amyotrophic lateral sclerosis, sarcoidosis, Crohn’s disease, ulcerative colitis.

Treatment with Izotec or its substitutes less than 6 months prior the procedure date.

Steroids, except asthma medications and inhaled steroids, taken less than one month prior to the scheduled procedure.

Fungal medications or antibiotics taken less than one week prior to the procedure.

Tendency to keloids.

Hemofilia.

Epilepsy.

Pregnancy and breastfeeding. We do not perform procedures in pregnant ladies or nursing mothers, even with the physician’s consent. When it comes to children, one can’t be too careful!

Asymptomatic HIV infection or active AIDS.

Skin diseases in the planned treatment site.

Fever, viral or bacterial infection on the day of the procedure.
